College Achieve Central Charter School treats the following signature like a signature on a paper form.

I verify the above information to be correct, and I understand that completion of this form does not guarantee placement in a school. I further understand that I cannot register my child without appropriate address documentation and that only one application can be submitted per child.

I further understand that it is my responsibility as the Parent/Legal Guardian to immediately inform the school of any changes to the information provided.

Please bring the following documentation upon acceptance:

  • Proof of residency (documentation requirement is dependent on resident district criteria):  property deed, mortgage statement, lease, current utility bill, *notarized statement from the landlord
  • Proof of guardianship (1 of the following): original birth certificate with raised seal (hospital birth certificate is not acceptable), copy of section of court decree awarding custody)
  • Proof of enrollment in local school district: child's report card and letter from district indicating you have registered
  • Proof of immunization: up-to-date immunization record from school or doctor
  • Proof of physical exam:  up-to-date physical examination from doctor

*notarized statement from landlord must be accompanied by another proof of residency

Kindergarten: Open Enrollment ends Thursday, February 19, 2026. (The Kindergarten lottery will be held Friday, February 20, 2026.)

Grades 1-12: Open Enrollment Ends Thursday, March 19, 2026. (Grades 1-12 lottery will be held Friday, March 20, 2026.)
