The following conditions allow parents to request a non-resident transfer. Please check all the reasons for the request. (3141P, Sec 3.4)

The following conditions allow parents to request a transfer to a school other than their boundary school. Please check all the reasons for the request.

I understand that a nonresident student will not be accepted if:

• The student has a history of convictions, violent or disruptive behavior, or gang membership; and/or
• The student has been expelled or suspended from a public school for more than ten consecutive days.
• Nonresident high school student athletes must meet the eligibility requirements of the Washington Interscholastic Activities Association (WIAA) in order to participate in varsity sports. 3141P, Sec. 4.0)

Parent/guardian understands that a transfer request may be revoked at any point for the following reason(s):

• Excessive tardiness or absences due to parent/guardian transportation issues (after reasonable steps have been taken to solve the issue).
• Increased enrollment at requested school causes school, grade or program to be in an overload condition.
• Conditions no longer exist that necessitated the transfer.

Furthermore, I agree that any placements are conditional and the transfer may be revoked based on the following conditions. If the student is excessively tardy or truant or if the student engages in frequent misconduct and/or disruptive behavior in violation of school rules [that indicates that the student is not being successful in the school or program. Should a decision be made to terminate the student's district enrollment, the parent/guardian will be notified and may appeal that termination to the Superintendent or his/her designee. 3141P, Sec. 3.0, 4.0, 5.0)]

A request for change in student assignment will not be approved for athletic or co-curricular activity reasons.  (As per WIAA regulations, students transferring from one high school to another high school within the Kent School District, without a corresponding  change in home residence with natural parents or legal guardians, will be deemed ineligible for a one year period at the varsity level of sports.)

I hereby certify that I have read all the conditions above and that the information provided on this form is true and correct to the best of my knowledge. I understand that approval of this request is dependent upon space availability at the requested school. For elementary openings, space availability in program, class size and staffing. For secondary openings based on school projections, staffing, and enrollment guidelines. I further agree that any placements are conditional and the transfer may be revoked based on the conditions above (3131P/3141P).

I understand that I will need to re-apply annually and that approval will be dependent upon all of the above conditions and a review of my student's school records for the previous year in the Kent School District. (3141P, Sec. 5.0)

I understand that once approved, the resident student transfer is effective for the duration of the student's career at the school in which the student transferred. Once the student completes the final grade in that school, the student will return to his/her home boundary school for the next grade. (3131P/3141P)

I understand that once approved, if the employee is a resident of the Kent School District the student transfer is effective for the duration of the student's career at the school in which the student transferred. Once the student completes the final grade in that school, the student will return to his/her home boundary school for the next grade. (3131P/3141P)

If the employee resides outside the Kent School District a new transfer application must be completed each year and parent/guardian must obtain a release from the district in which they reside.

I understand that a child admitted under false information is illegally enrolled and could result in disenrollment upon discovery. I further understand that it is my responsibility as the Parent/Legal Guardian to immediately inform the district of any changes to the information provided.
