Open Enrollment Request Form 2025-2026

SUNNYVALE SCHOOL DISTRICT

February 3, 2025 through February 14, 2025

Students residing within the district boundaries entering grades TK-8th during the 25-26 school year have an opportunity to apply and attend another school within the district other than their home school.

You must complete Registration for the 25-26 school year or be a current student to apply for Open Enrollment.

Sibling Information

A resident of the school you are requesting cannot be displaced by your transfer. If the grade becomes full, your student may be returned to their home school within two weeks of school starting.

  • Parents/guardians will be notified of their results and drawing order in the lottery during April via email.
  • If a seat is not offered once school starts, the student will attend their home school, and may reapply for Open Enrollment next year.
  • Approved Open Enrollments do not have to apply in subsequent years.
  • Transportation is NOT provided to student on Open Enrollment and will not continue if your student was previously an overflow to another school.

I hereby certify that I have read all the conditions above and that the information provided on this form is true and correct to the best of my knowledge. I understand that approval of this request is dependent upon space availability at the requested school. For elementary openings it is based on space availability in program, class size and staffing. For secondary openings it is based on school projections, staffing, and enrollment guidelines. I further agree that any placements are conditional and the transfer may be revoked based on the conditions above.

I understand that a child admitted under false information is illegally enrolled and could result in disenrollment upon discovery. I further understand that it is my responsibility as the Parent/Legal Guardian to immediately inform the district of any changes to the information provided.
